Please note that both pages as in attachment have to be filled duly attested by the shipper and stamped and signed

The documents as stated on the 2nd page of the KYC have to be duly stamped and signed and handed over

This is a 1 time Registration with Jeena – and you will not be required to send them again if your packages are cleared by Jeena
